Your health is important.

Old National values all of our team members as important team members, which is why we believe their health is important. Partnering with our team members to improve our corporate health includes the development of our employee wellness program, ONB Well.

As a benefit to all team members, ONB Well has partnered with Deaconess at Work to offer a full range of health enhancement activities and services, exhibiting Old National’s commitment to help associates achieve better health. Included are fun wellness activities available to all associates as well as targeted programs focused on helping those with specific health conditions.

The comprehensive program features access to a personal health coach, health screenings, online programs, personal and Wellness Challenges, a personalized Web site and mobile App, and much more. Associates may access the website at any time to be in touch with the most up-to-date wellness information.

Incentive Program

2023 Premium discount: Complete a Biometric Screening and complete the personal health survey on your Deaconess Wellness Portal to earn your 2023 premium discount.

  • Wellness Discount - Earn a $50 discount per month by completing the following two (2) wellness activities:

    1. Complete an online Health Risk Assessment

      • To take your Health Risk Assessment follow these steps:

        • Go to the Deaconess at Work portal at https://onb.personalhealthportal.net/login. 

        • Instructions to register for the portal can be found by clicking here.

        • Once you are logged in to the portal access the Health Risk Assessment by clicking on “Take Survey” button located in the Survey Status section on the home page. 

    2. Complete a Biometric Screening

      • Biometric screenings may be completed through Deaconess - Deaconess Clinic at Work - ONB or through your physician.

      • Team members located in the Evansville area may schedule a biometric screening through Deaconess - Deaconess Clinic at Work - ONB.

      • The physician form may be downloaded from the Deaconess at Work Portal at https://onb.personalhealthportal.net/login.

        • To access the form, click on the Wellness Screenings quick link on the left of the home screen and then on the “Physician Screening Form”

      • If you have had labs performed at your physician’s office from January 2023 to present day, these can be used for your biometric screening incentive.

      • Download the Physician Screening Form to have your physician complete. Your physician can either fax back to the fax number on the form or you may email it to Madison.Petrig@deaconess.com

      • Incentive will be given on a go forward basis once wellness program criteria has been met.

         

  • Tobacco Free Discount – Earn a $50 discount per month for being tobacco-free or by completing a tobacco cessation program. You will certify your tobacco free or tobacco user status during open enrollment.

  • To complete the tobacco cessation program, call the Deaconess health coach at 812-492-5717. You may also complete a tobacco cessation program through another provider, please contact HR Services for additional information.

  • Privacy Statement: All personal health information recorded online is stored on the secure servers of Activate Health, the company that administers the Wellness Assessment and operates the Web site. No one at Old National will have access to associates’ protected health information. Associates’ privacy is assured when they take the Wellness Assessment. Only aggregate data will be provided to Old National to identify trends and more accurately target wellness activities for the population as a whole.
