The VSP provider network includes major eye care centers nationwide.

Full-time and Part-time Level I team members may elect to enroll in vision care benefits provided through Vision Service Plan (VSP). For a list of participating eye care centers near you, visit vsp.com or call (800) 877-7195.

With VSP, you can have your eyesight examined and purchase frames and lenses at a small co-payment when you go to optometrists, ophthalmologists and opticians who participate in the VSP plan.

How To Use Your VSP Benefits

ID cards will not be issued for this plan.

  1. Call your VSP doctor and make an appointment.

  2. Provide the doctor’s office with the following information:

    • Patient’s name and birth date;

    • Name of the company that provides your VSP coverage (Old National); and

    • Team member’s VSP member identification number (Social Security Number).

  3. The doctor will check your eligibility for services and plan coverage.

  4. Pay your VSP doctor for any co-payments and other costs not covered by your VSP plan.

VSP’s Participating Provider List

To obtain the highest benefits of the plan, you need to select a participating provider to receive coverage.

Out-Of-Network Providers

For services from an Out-Of-Network provider, VSP will reimburse you up to the amount allowed under the plan's Out-Of-Network provider reimbursement rate. Please remember that the Out-Of-Network provider reimbursement rate does not guarantee full payment. Please contact VSP’s customer service department for details.

Under the provisions of the vision plan, you will pay the entire bill when you see the Out-Of-Network provider and gather the following information:

  • Provider’s bill, including a detailed list of the services you received;

  • Associate’s VSP member identification number (associate’s Social Security Number);

  • Associate’s name, phone number and address;

  • Name of the company that provides your VSP coverage (Old National);

  • Patient’s name, birth date, phone number and address; and

  • Associate’s relationship to the covered VSP member (such as self, spouse, child,).

Claims must be filed with VSP within six months of seeing the provider.

To file a claim, you must sign into the VSP website. Click on Log In/Registration along the top. You must either log in or register to access your information. Once you have entered VSP’s website, you will see your benefits information. Scroll down to the bottom of the page and click on “Out-of-Network Reimbursement.”  Then, follow the instructions to complete your request. Please remember to keep a copy for your records.

If you are enrolled in this benefit, you may view your current coverage by accessing the ADP mobile app or MyONB portal. The portal may be accessed from the home page of the intranet or at myADP.com. Once you log onto the portal, select Benefits from along the top of the screen, then Health & Welfare. Select View Benefits on the Current Benefits card to access your Benefits Summary.
