Benefit-eligible team members can make changes to their benefit elections outside the open enrollment period if they have a change in family status.

Family Status Change Examples

Examples of family status change events include:

  • Marriage, divorce, legal separation, annulment

  • Domestic partnership or termination of domestic partnership

  • The birth or adoption of an team member's child or their domestic partner's child

  • The death of an team member's spouse/domestic partner or dependent

  • Spouse/domestic partner becoming eligible or ineligible for coverage (for example, a spouse/domestic partner's open enrollment period)

  • Change in employment status from part-time to full-time status or full-time to part-time status

  • Change in part-time status level (benefit eligible vs. non-benefit eligible)

  • An unpaid leave of absence for an team member or team member's spouse/domestic partner

  • Significant change in the group health coverage provided by your spouse/domestic partner's employer

  • Change in dependent's eligibility for coverage (for example, when a dependent reaches the maximum age limit)

Benefit Changes Allowed

When a family status change takes place, you may adjust the following coverages:

  • Coverage level under the Medical, Dental and/or Vision plan (you may only change your medical or dental plan option during the annual enrollment period)

  • Supplemental and/or Dependent Life Insurance coverage

  • Voluntary AD&D Insurance coverage

  • Critical Illness Insurance coverage

  • Supplemental Accident Plan coverage

  • Hospital Indemnity coverage

  • Flexible Spending Account (including Commuter) contributions

You may only make changes that are consistent with the change in status. For example, if you get married, you can add your spouse to you coverage or drop your coverage to enroll in your spouse's plan.

Employees' Savings (401(k)) Plan contributions and elections can be stopped or changed at any time by calling the Empower’s Benefit Line, at 1-844-465-4455 or via online at myonbretirement.com. The contract number is 150577-01.

Family Status Change Requests

You may visit the MyONB Portal, use the ADP Mobile app or contact the Benefits Solution Center at 1-800-240-7155 to make changes to your benefits elections resulting from a family status change event. After you log into the Portal, click on "Benefits” and then “Health & Welfare.” Select "Declare an Event" on the Report a Qualifying Change card. You must identify the family status change event that pertains to your situation prior to changing your benefit election(s). The request for benefit election changes must be submitted within 31 days of the family status change event. If documentation of the status change is required, you will be contacted by the Old National Benefits Solution Center. Supporting documentation must be received within 31 days of the receipt of your Family Status Change request. All requests received outside of the 31 day period will not be permitted due to IRS requirements.
