Accidental Death & Dismemberment

Through the voluntary accident insurance plan, Full-time and Part-time Level I team members may purchase additional accidental death and dismemberment (AD&D) coverage similar to the Basic Life AD&D coverage. If you elect this benefit, it will cover you or you and your family should you die due to an accident.

You may choose coverage in various amounts up to $1,000,000 or ten times your salary, whichever is less. Coverage for your spouse (if elected) is equal to 60% percent of your benefit amount without children; 50% of your benefit amount with children. Coverage for all your dependent children (if elected) is equal to 10% of your benefit amount with covered spouse; 15% of your benefit amount without a covered spouse.

The payable Voluntary AD&D benefit will be reduced upon attaining the following ages:

• age 70, 35% of the original coverage amount.

• age 75, 55% of the original coverage amount.

• age 80, 70% of the original coverage amount.

Life Insurance Beneficiaries

You can designate beneficiaries for the Basic Life AD&D, Supplemental Life, and Voluntary AD&D insurance plans online via the MyONB Portal at any time.

If you are enrolled in this benefit, you may view your current coverage by accessing the MyONB portal. The portal may be accessed from the home page of the intranet or at myADP.com. Once you log onto the portal, select Benefits from along the top of the screen, then Health & Welfare. Select View Benefits on the Current Benefits card to to access your Benefits Summary.
