A High Deductible Health Plan (HDHP) requires the enrollee to pay for most medical and all prescription drug expenses up to a certain dollar amount before the plan begins to cover them.  It is intended to protect you should you experience a major (or catastrophic) health care event.

High Deductible Health Plan

The HSA-HDHP 2250, HSA-HDHP 3500 and HSA-HDHP 4000 plans will begin paying covered network medical and prescription drug expenses at 80% once your out-of-pocket medical and prescription drug expenses reach the appropriate deductible level. The exception to this involves coverage for preventive care. Preventive care is covered at 100% even if you have not met your full deductible. Once the maximum out-of-pocket has been satisfied, the plan will pay for all eligible medical and prescription expenses at 100%. 

Under the HSA-HDHP 2250 plan, the family deductible is a non-embedded, or aggregate, deductible. This means that if you are enrolled in a level of coverage other than single, the entire family deductible must be satisfied (other than for preventive care) before any eligible expenses will be paid by the plan.

If you are enrolled in the HSA-HDHP 3500 or HSA-HDHP 4000 for coverage other than single, the deductible and out-of-pocket amounts can be satisfied by any combination of family members but an individual would never satisfy more than their own individual amount (embedded).

Health Savings Accounts (HSA)

A Health Savings Account is a type of account available for only those who enroll in a High Deductible Health Plan. The account is used to pay for eligible expenses not covered by a HDHP. Contributions to an HSA may be made by the team member, employer or both. Old National will make a contribution to eligible associate HSAs as soon as administratively possible after the effective date of coverage. Eligible team members enrolled in a HDHP and ONB HSA with single coverage effective January 1 will receive a $500 HSA contribution from Old National. Team Members with any other level of coverage effective January 1 will receive an $1,000 contribution. The timing of the company provided HSA contribution will occur quarterly in four equal installments at the beginning of January, April, July, and October. Please note the ONB contribution counts towards your annual maximum IRS HSA contribution limit.

The 2025 annual maximum allowable contribution to an HSA, as established by the IRS, is:

  • Individual - $4,300

  • Family - $8,550

Individuals 55 and older can make an additional catch-up contribution of $1,000 in 2024 and 2025.

Frequent Questions Regarding HSA

Eligible team members may start, stop, or change their HSA election at any time during the plan year, with or without a qualifying life event.  Except for holiday weeks, changes to your HSA deduction amount must be made by Wednesday of a non-payroll week for the changes to be effective with your next pay. Changes can be made either through the MyONB Portal, the ADP mobile app, or by calling the Benefits Solutions Center at 1-800-240-7155. 

When making the change in the MyONB Portal, on the main menu: 

  1. Select “Benefits”

  2. Select “Health & Welfare”

  3. Look for the box titled “Report a Qualifying Change” and select “Declare an Event”

  4. Select HSA Election and enter the current date

  5. Select “Change my yearly goal amount” and enter your new HSA amount (Either annual goal or per pay amount). If you are wanting to stop your current contribution, select “Waive Coverage” at the bottom of the page.

  6. Select “Save and checkout”

  7. Review your election

  8. Select “Confirm Enrollment”

HSA Application & Instructions

Team members who enroll in a High Deductible Health Plan must complete the online HSA application with The UMB Health Services to receive the company’s annual contribution to their HSA.

Applications not received in a timely manner will experience delays in receiving the company's contribution, your pre-tax payroll contribution(s) and/or Debit Card. The Debit Card can be used to pay for qualified expenses directly from your account.

If you elect High Deductible Health Plan coverage during Open Enrollment and do not have an ONB Health Savings Account, you must submit your Health Savings Account application online to UMB by December 20, 2024, to avoid delays in receiving the company's contribution, your personal contribution and/or Debit Card for the HSA.

How to Open Your HSA Account

  1. Go to hsa.umb.com. Click Open an HSA under the individual tab.

  2. Select the option If you have been instructed by your employer... When prompted, enter your six-digit employer code listed below.

  3. Click Continue and complete enrollment. Submit the application and you’ll receive a confirmation number.

  4. A welcome letter will arrive in the mail within 10 business days of your application and should be retained for your records.

  5. If you requested a debit card, it will be mailed separately. If checks are requested, the order is held and processed after your balance reaches $25.00.
